Provide the information in the form above, indicate Division & date of tournament you are applying for and include a check or money order in U.S. funds made out to: Salmon Hockey Association. 4 games guaranteed. Applications open September 1. DO NOT SEND MONEY or PRIOR YEARS FORMS PRIOR TO September 1 as our Tournament Coordinator cannot accept applications and checks prior to this date.
